Patient referred to the Social prescribing Service for Mental wellbeing support and work. Patient fell from a height which left him in a wheelchair at 22 and came to the SP service not knowing if he would be able to walk again. Patient was seemed very low having to have careers to assist and moving back into family home which patient stated he was struggling with as ‘have always been independent.’ Patient used to have a job at a food store/delivery but had to give this up and found himself bed ridden and unable to do the things he used to do. Patient stated he is feeling his mental health declining as well as physical health as wanted to get some support with this and to ‘keep my mind busy and focused’ We discussed many things as I tried to get patient back on a positive track and find things he could do whilst still being at home. From this, the patient decided to do some online learning as he stated he always wanted to be a paramedic but never had the to study. I signposted patient to South West skills academy/Reed courses and National Careers organisation and also made a referral to the Positive People Project. I also signposted patient to counseling/Man Down support group. Within 6 weeks I received this email from patient.

‘Hello, hope you're doing ok. Just writing to let you know that I do not think I will be requiring any more help from CHAOS, I am in a position now where I am a lot more independent than I was, now that I'm on my feet I'm starting to get into the rhythm of things again - I have enrolled myself in some courses and have a somewhat clear view of where I am to go moving forwards, with all the support I need to do so! Hope this is OK, would like to say thank you for all the kindness and support you have given me. JT.'
From this, I called patient to get final wellbeing scores and to congratulate him on his positive changes and the fact he’s been receiving physio support to get him back on his feet.
